An unannounced monitoring inspection was conducted today between 12:15pm-1:05pm. There were 5 children (15 points) present with the Provider and 1 assistant within the supervision guidelines. The children were observed taking a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Evidence:
1. Child #2 (doe: 6/6/2023) did not have 1 of 2 required emergency contact's information (name, address, phone number) on file at the time of the inspection.Plan of Correction: I will ask the parent for another emergency contact.
Standard #: 8VAC20-800-90-A Description: Based on a review of children's records, 1 out of 3 children's records did not have documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. Child #1 (doe: 4/15/2024) did not have documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations at the time of the inspection.Plan of Correction: I will ask the parents to bring the immunizations in for the child.
