An unannounced monitoring inspection was conducted on May 31 2024 between 10:15am and 11:00am. There were 2 children in the care of the provider. Children were observed having pretend play, being fed, and singing. Household records, the provider record, child records, pet records, and the physical space was reviewed and inspected. Standard #: 8VAC20-800-90-C Description: Based on record review, the provider did not ensure that additional immunizations for a child were obtained.
Evidence: Child #1 record did not have updated immunizations every 6 months from birth to 2 years of age. Child is under two, and the immunization record on file is dated September 28 2023.Plan of Correction: Not available online. Contact Inspector for more information.
