An unannounced monitoring visit and facility tour was conducted on 11/16/2023. At the time of inspectors' arrival there were 7preschool aged children in care with 1 staff members and 2 administrators. Children were observed interacting with staff, nap, lunch and hand washing. A sample of 5 children's records and 3 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22.1-289.035-A Description: Based on record review and interview it was determined the center did not ensure that a repeat criminal history check and central registry search was performed for each staff member at least every five years from the date of the last search.
Evidence:
1. The record for staff #1(hired 8/1/16) did not contain documentation of an updated criminal history check. The documentation available was dated 6/25/18.
2. The record for staff #2(hired 7/17/16) did not contain documentation of an updated criminal history check or central registry search. The documentation available was dated 6/14/18 and 8/7/18.
3. The record for staff #3(hired 9/8/16) did not contain documentation of an updated criminal history check. The documentation available was dated 11/19/18.

Standard #: 8VAC20-780-560-F Description: Based on observation and interview it was determined the center did not ensure a menu listing foods to be served for meals and snacks during the current one-week period was posted in a location conspicuous to parents or given to parents.
