During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Effective January 1, 2025, VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 11/18/2024, as a part of the licensure period. The on-site inspection began at 11:20am and ended at 3pm. The inspector reviewed compliance in the areas listed above. There were 66 children present and 10 staff. The inspector reviewed 6 children?s records and 6 staff records on-site.
This inspection included document review. tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment. The central registry search request for Staff #1 was requested 2 weeks after Staff #1's first day of employment.

Plan of Correction: The required central registry check was obtained and the results are in the staff's record. Requests for a search of the central registry will be submitted prior to staff's first date of employment.

Standard #: 22.1-289.035-B-4
Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #4, employed for more than 9 months, did not have documentation of requesting a central registry check from the state Staff #4 identified as a state in which they resided in the past 5 years.

Plan of Correction: The center will provide documentation to the Department that the required background check was requested.

Standard #: 8VAC20-780-150-B
Description: Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #3 and child #4 was not signed and dated.

Plan of Correction: The center will make sure immunization documents received are signed and dated.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2, enrolled for two months, was missing the home address of each parent who has custody.

Plan of Correction: The assistant director emailed the parent during inspection to request the child and parents' current address.

Standard #: 8VAC20-780-240-E
Description: Staff are required to complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment. The training for Staff #1 was completed two weeks past the required date for completion.

Plan of Correction: The training has been completed and documentation is in the staff's record. All staff will receive first aid and cpr training within 30 days of hire.

Standard #: 8VAC20-780-340-D
Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #1, who did not meet lead teacher qualifications, was alone with a group of preschool children and was the only assigned staff member in the classroom on the day of the inspection.

Plan of Correction: The center will work with staff to become lead teacher qualified. The center will make sure a lead qualified staff is with each grouping of children.

Standard #: 8VAC20-780-550-D
Description: The center is required to implement a monthly practice evacuation drill. The center did not practice evacuation drills during the months of August 2024 and September 2024.

Plan of Correction: The center will complete two additional practice evacuation drills and will post the drill log on the wall so that it is more visible as a reminder to complete the drills.
