A complaint inspection was conducted on March 7, 2024, from 9:20am to approximately 12:30pm in response to a complaint that was received by the department on March 1, 2024, regarding allegations in the care of the children at the center. Staff were interviewed, documentation was reviewed, classroom observations were completed, video of the incident was reviewed, text messages were reviewed between director and parents, and a tour of the facility was conducted. Information gathered during the inspection determined non-compliances with applicable standards or law and violation a was documented on the violation notice issued to the program. The complaint is determined to be valid for not having the appropriate documentation from the incident. The violation was discussed with the owner/director at the exit interview.

Standard #: 8VAC20-780-550-P Complaint related: Yes Description: Based on record review and interview, it was determined that the center did not ensure that they maintain a written record of children's serious and minor injuries
in which entries are made the day of occurrence.
Evidence:
1. On February 28, 2024, a child at the facility obtained a bloody nose when pushed by another child.
2. Staff #1 confirmed that there was no injury report document completed on the day of the incident, February 28, 2024.Plan of Correction: The center responded with the following: The center will ensure that injury reports are not only documented via text or the Brightwheel app, but on the DOE model form for injury reports. The written reports will be placed in the child's record once signed by staff and/or parent.
