Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on 1/14/2021 and concluded on 1/15/2021. The director was contacted by telephone to initiate the inspection. There were 25 children present and 5 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility. Description:	Based on record review and staff interview, the center did not have a written allergy care plan on file for each child with a diagnosed food allergy. Evidence: Child #4, Child #5, and Child #6 have a food allergy and did not have a written allergy care plan on file.
Plan of Correction:	For Child 4 and 6 - Task Complete. Child #5 is no longer enrolled as of 1/19/21.
Standard #:	22VAC40-665-580-B
Description:	Based on record review and staff interview, the center did not ensure that all staff who work directly with children have completed Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: Staff #3 (Date of Hire: 10/25/2011) did not have documentation of Preservice training.
