An unannounced renewal inspection was conducted on site on 1/8/2024 from 6:30am to 7:50am and concluded on 1/11/2024 with additional remote file review. There were 13 children directly supervised by 3 staff. The physical plant, programming, menu, 3 staff records, 5 children?s records, 1 medication, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in quiet table top activities. Positive interactions between children and staff were observed during this inspection. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.035-A Description: Based on record review, the center did not obtain updated fingerprint-based criminal history background checks for all staff every 5 years.
Evidence: Staff #1's most recent fingerprints on file were completed 9/28/2018.Plan of Correction: Staff went for fingerprinting.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas and equipment inside the center was not maintained in a clean, safe, and operable condition.
Evidence:
1. In the upstairs girls bathroom there was a drain in the floor with no cover leaving a four inch hole on the ground next to the toilet.
2. In the upstairs recreational area there was a pool table that was missing one of the four feet and the leg was not touching the ground. Due to the missing foot, the heavy pool table was unbalanced and there is a significant risk of it falling on a child with any applied pressure.Plan of Correction: Drain cover installed.
