A complaint was received on September 20, 2023 by the licensing office regarding communication and forbidden actions. An on-site investigation was initiated on September 28, 2023. Interviews were conducted and records were reviewed. Video footage was reviewed on October 17, 2023. Based on the preponderance of evidence, the complaint is deemed valid. Violations cited can be found on the violation notice. An unrelated violation was cited and is identified on the violation notice. Standard #: 8VAC20-780-350-B-4 Complaint related: No Description: Based on video footage review, the center failed to maintain staff-to-children ratios whenever children were in care.
Evidence:
1. On September 18, 2023, video footage shows 12 children in the 3-year-old Classroom with Staff #1 from approximately 8:37 a.m. to 8:41 a.m., exceeding the 10:1 ratio for three to five year olds.
2. Staff #2 arrived to the classroom at approximately 8:41 a.m. and left with two children, leaving Staff #1 with 10 children, within ratio.
3. Video footage shows 3 additional children getting dropped off in the classroom between approximately 8:50 a.m. and 8:57 a.m. leaving Staff #1 with 13 children, exceeding the 10:1 ratio, until Staff #3 arrived at approximately 9:08 a.m.Plan of Correction: Not available online.
