A Subsidy Health and Safety inspection was conducted on April 19, 2024, to review supplemental health and safety requirements. During a self reported investigation that was received by the licensing office on March 19, 2024, an investigation was initiated in regards to a violation of the vendor agreement. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 8VAC20-790-520-A Description: Based on interview, the vendor did not ensure compliance with the terms of the vendor agreement.
Evidence:
1) The Vendor Agreement states, ?The Vendor shall not be in possession of or swipe any parents? or guardians? VA-ECC card.?
2) Witness #1 confirmed that the center has possession of parents VA-ECC cards.
3) Witness #1 stated that the VA-ECC cards are kept in a room that the parents do not have access to.
4) Staff #1 stated that Staff #2 has been in possession of VA-ECC cards.
5) Staff #1 stated that they have seen Staff #2 help parents swipe the VA-ECC cards.
6) Staff #2 confirmed that the center has had possession of 2 parents VA-ECC cards.
7) Staff #2 confirmed that the center no longer has possession of any parents' VA-ECC cards.Plan of Correction: Not available online. Contact Inspector for more information.
