Areas of non-compliance are identified in the violation notice. Please contact me if you have any questions at Karen.dickens@doe.virginia.gov.

Violations:
Standard #: 8VAC20-800-700-A
Description: Based on record review and interview, the provider did not have written authorization from the child's physician and parent for a long term prescription medication.
Evidence:
1. Child #4 has a long term prescription medication on site without a medication authorization form completed and signed by the parent and the child's physician.
