An unannounced renewal inspection was conducted on February 11, 2020, from 2:50pm to 4:46pm to determine the provider's compliance with licensing standards. The children were waking up from their nap upon the inspector?s arrival, and they were later observed in free play. Today, there were six children in care (one resident child), totaling eight points, with one caregiver.
Interviews were held with the caregiver throughout the inspection. Additionally, when appropriate, the inspector interacted with the children. The caregiver was observed having positive interactions with the children. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Lunch today consisted of turkey, rice, green beans, applesauce, bread, and milk, which was a change from the posted menu.
The kitchen, bathroom, and designated childcare areas were inspected during today's inspection. The inspector observed the required postings. The first aid kit and non-emergency medical supplies, fire and shelter-in-place drills were reviewed. Seven children's records and one caregiver record were reviewed.
Per the provider, medication is not administered and transportation is not provided at this time.

The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection.

Standard #: 22VAC40-111-730-D Description: Description: Based on observation, the provider did not ensure medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, were kept in a locked place using a safe locking method that prevents access by children.
Evidence: There was an unlocked medicine cabinet, containing several medications, in the caregiver's bedroom. The bedroom door was not locked. The bedroom could also be accessed through the bathroom used by children, and that door was also unlocked.Plan of Correction: I will put a lock on it.
Standard #: 22VAC40-111-780-B Description: Description: Based on review of children's records, the provider did not ensure that annually, a review with the parent of the emergency contact information is conducted to ensure the information is correct.
Evidence: The record of Child #5 did not contain documentation of the review of the child's emergency contact information. The child's record was last reviewed 03/21/18.Plan of Correction: Will get parents to do by 2/17/2020.
Standard #: 22VAC40-111-920 Description: Based on observation, the provider did not ensure the menu contained all required information.
Evidence: The posted menu was dated 2016.Plan of Correction: I will put up an updated menu.
Standard #: 22VAC40-111-100-A Description: Description: Based on a review of records, the provider did not obtain a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: 1. The record for Child #1(DOE 10/16/17) did not contain documentation of a physical examination.
2. The provider acknowledged there was no documentation of a physical examination in Child #1's record.Plan of Correction: Will get parents to bring by 2/17/2020.
Standard #: 22VAC40-111-40-A Description: Based on observation, the provider did not ensure that the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.
Evidence: 1. Upon the arrival of the inspection on February 11, 2020, at approximately 2:50pm there were two children in care.
2. At approximately 3:20pm, an additional five children arrived at the home for a total of seven children, exceeding the family day home's capacity of five children.Plan of Correction: I will make sure I only have 5 children at one time.

Standard #: 22VAC40-111-70-A Description: Description: Based on record review on February 11, 2020, the provider did not ensure to obtain the parent's signed written acknowledgement of the receipt of the information for parents.
Evidence: 1. The record of Child #3 and Child #5 did not contain documentation of signed acknowledgement of information for parents.

Standard #: 22VAC40-111-280 Description: Description: Based on observation, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited
to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates were stored away from food in areas inaccessible to
children.
Evidence: A container of Lysol wipes was observed on the floor in the area where children receive care. The label on the Lysol wipes contained the language "caution" and "keep out of reach of children."Plan of Correction: I will move them to be inaccessible.
