The license cannot be processed for renewal until the fully completed renewal application and required fee are received.

An unannounced full renewal inspection was conducted today. There were 40 children in care, with a total of 9 staff members assigned to their direct supervision. Children's medications, children's records, the physical plant, classroom supervision, programming, staff records, staff training, and emergency preparedness items were inspected today.
The children were observed enjoying circle time activities, playing with toys, completing academic activities, eating lunch from home, and taking their afternoon nap.

Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that one record does not include all required information.
Evidence:
Child record A does not include the information for a second emergency contact.Plan of Correction: The parent will be reminded to update the information in the file.
Standard #: 22VAC40-185-80-A Description: Based on a review of administrative documents, it was determined that the written record of attendance was not correct in one classroom.
Evidence:
In the Toddler classroom, a total of 8 children were present and in care. However, the record of attendance indicated a total of only 7 children in care.Plan of Correction: The 8th child just transitioned to that classroom. Her name will be added to the correct attendance.
Standard #: 22VAC40-185-440-N Description: Based on observations, it was determined that two cribs used by infants over the age of 5 months were equipped with toys hanging over the sleeping area.
Evidence:
Two cribs in the infant classroom were equipped with mobiles attached to the side bars and extending over the sleeping area of the crib mattress. The infants that use these cribs are both age 6 months.Plan of Correction: The mobiles were removed during the inspection.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that two medications were on site without all authorization requirements.
Evidence:
Two medication authorization forms expired on 1/8/2020.Plan of Correction: The parent will be asked to bring new authorization forms.
Standard #: 22VAC40-185-510-J Description: Based on observations, it was determined that one child's medication was not kept in a locked place using a safe locking method.
Evidence:
The prescription medication for one child was in the child's bag. This bag was hanging on the child's cubby hook in the hallway. This method of storage does not keep the medication locked.Plan of Correction: The medication was immediately moved to a locked location during the inspection.
Standard #: 22VAC40-185-550-M Description: Based on a review of completed injury report records, it was determined that some of the records do not include all required information.
Evidence:
After a random review of children's injury reports, a total of 5 reports did not include a second adult signature. These reports only included the signature of one staff person.Plan of Correction: Staff will be reminded to include a second signature, either the parent or a staff person, by the end of the day when an injury report is filled out.
