A subsidy health and safety inspection was conducted on 8/16/2024 between 10:30am and 11:15am. There were three children receiving care by the vendor. Previous violations, the physical plant, polices and procedures, emergency supplies and procedures, emergency drills, 3 children's records, the vendor's record, and 1 household member's record were reviewed. An exit interview was conducted with the vendor. Areas of non-compliance can be found on the violation notice.

Standard #: 8VAC20-790-230-B Description: Based on record review and interview, the vendor did not obtain documentation of additional immunizations once every six months for children less than the age of two years.
Evidence: Child #1 (15 months old), date of last immunization 11/2023.Plan of Correction: Vendor to request information from parent today.
Standard #: 8VAC20-790-250-F Description: Based on documentation review and interview, the caregiver who works directly with children did not annually attend the department's health and safety update course.
Evidence:
1. Date of last health and safety training certificate 10/2023.
2. Vendor stated they didn't know it was every year.Plan of Correction: Vendor will attend training.
