An unannounced inspection was initiated with the local department of social services and law enforcement on 7/30/3024 and completed on 9/24/2024 in response to a complaint received by the licensing office on 7/26/2024 relating to allegations of a forbidden action and parental involvement. The inspector reviewed 1 child record and 2 staff records on-site on 7/30/2024. This inspection included document review, a tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined noncompliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/1/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: The following actions are forbidden: restricting movement through binding or tying, forcing a child to assume an uncomfortable position, enclosure in a space that the child cannot freely exit himself.
On 7/16/2024 staff #1 engaged in restraining child #1 so that they assumed an uncomfortable position and their movement was restricted for 1-2 minutes.Plan of Correction: child #1 was attaching another child by striking them in the upper body/face. The teacher placed child #1 in a protective bear hug per our family handbook. Upon learning that this violates VA standards, we are rewording our family handbook to align with VA standards.
Standard #: 8VAC20-780-420-B Complaint related: Yes Description: Staff are required to notify parents of disciplinary steps taken in response to persistent behavior problems.
On 7/16/2024 staff notified parents of behavior issues but did not notify them of disciplinary steps taken.Plan of Correction: Parent was verbally notified of said action, but it was not documented on the observation report. Moving forward we will document on written report.
