An unannounced non-monitoring focus inspection was conducted on February 6, 2020 from approximately 8:15 am -9:35 am.
Children were observed arriving and participating in center activities. There were nine children present and two staff. Five children records and three staff records were reviewed. The inspection will be reviewed with the administrator to determine early compliance.

Standard #: 22VAC40-185-140-A Description: Based on review, facility did not ensure 1 of 5 children's records has a physical within a month of beginning attendance.
Evidence: The record for child #4 ( start date August 13, 2018) did not have a physical.Plan of Correction: The center will continue to locate the physical in the facility. The parent was called and message was left during the inspection. The center will send physical as soon as it is received.
