An unannounced inspection was initiated on 9/13/2024 and completed on 9/13/2024 in response to a complaint received by the licensing office on 9/10/2024 relating to allegations of lack of supervision. The inspector reviewed three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to bottles and special feeding needs and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/27/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-280-B Complaint related: No Description: Hazardous substances should be kept in a locked place that prevents access by children.
Evidence: Lysol bottle and fragrance spray with statement "keep out of reach of children" was left on shelf in infant room unlocked and accessible.Plan of Correction: Lysol spray and fragrance was not in reach of infants however staff was informed that at no time should these things be left out.
Standard #: 8VAC20-780-570-C Complaint related: No Description: A record for each child should contain the brand of formula and the feeding schedule of the child.
Evidence: Staff did not have an up to date record for any children in the infant room. Records were improperly dated and did not have up to date information written.Plan of Correction: Staff explained that the date was incorrect, however they now know that it is important to have accurate information
Standard #: 8VAC20-780-570-E Complaint related: No Description: Bottles with infant formula should be refrigerated, dated, and labeled with all children's names.
Evidence: Five bottles were observed unlabeled and not dated in the infant room.Plan of Correction: Staff in infant room will make sure that all bottles and cups are labeled and dated.
