During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Standard 8VAC20-780-(6)-360-A was reviewed.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated and completed on 9/28/2024 as a part of the provisional licensure period, The on-site inspection began at 1:30pm and ended at 4pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/20/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-F Description: Children under 10 years of age always shall be within actual sight and sound
supervision of staff. From approximately 2pm through 2:30pm Staff #1 and #2 left a group of 4 to 8 preschool children unattended, no less than three times. The children were awake and moving around the room.Plan of Correction: Staff #1 and Staff #2 remained in the classroom with the children. A review of sight and sound and training will be completed for all staff members.
Standard #: 8VAC20-780-400-B Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts. Staff #1 was heard saying ?go lay yourself down! Jesus! Staff #1 was speaking to two preschool children. Plan of Correction: Training will be provided to all staff on proper communication with students and the use of negative mood words.
Standard #: 8VAC20-780-440-E Description: There shall be at least 12 inches of space between occupied cots. Child #1 and #2 were sleeping/resting on cots that had less than 12 inches between them. Plan of Correction: Staff will be trained on using a 12 inch ruler daily during nap time to ensure all children are 12 inches a part.
Standard #: 8VAC20-780-450-A Description: Cots used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Child #3 was sleeping/resting on a cot without a top or bottom cover. Plan of Correction: Children will not be permitted to enter the center on Mondays if they do not arrive with all of the required napping linens.
Standard #: 8VAC20-780-500-A Description: Staff shall wash their hands with soap and running water before and after helping a child use the toilet and before and after a diaper change. Staff #1 assisted 11 preschool children with toileting and diaper changing without washing her hands between each child. Plan of Correction: Training will be provided on proper handwashing and diapering procedures.
