This inspection was conducted by licensing staff using an alternate on-site and remote protocol, including telephone contacts, documents review, interviews, and on-site observation of the program.
A hybrid monitoring inspection was conducted on 01/14/2022 regarding a possible self-reported incident. During the on-site visit, there were 36 children present, ranging in ages from 4 months through five years, with 7 staff supervising. The inspector reviewed compliance in the areas of physical plant and staffing and supervision. Also on 01/14/2022, a virtual remote inspection was conducted were two children's records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with a non-incident related standard and the violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E Description: Based on the review of two children's records and verification from staff, the center failed to obtain an updated immunization record every six months for a child under two years of age.
Evidence:
1. The record for Child #1 did not contain one of the updated immunization records which are required every six months for children under the age of two. There was no updated immunization record from 06/18/2018 to 09/11/2019. Date of enrolled for Child #1 was 04/17/2018.
2. Staff #1 confirmed that there was one immunization record which had not been obtained.Plan of Correction: This was an oversight. The staff understands that they need to get an updated immunization record which is signed by a physician or his designee every six months from the date of enrollment until a child turns two years of age.
