An unannounced monitoring inspection was conducted on 04/23/2024 from 10:45 a.m. to 11:40 a.m. There were nine children present, ranging in ages from ten months to three years old with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-80-A Description: Based on interview and review of records, the provider failed to obtain verification of identity within seven days of attendance.
Evidence:
1. Child # 1?s first day of attendance was 07/21/2023 and there was no proof of identification on file for Child #1. The provider confirmed that proof of identification had not been obtain within seven days of attendance for Child #1.Plan of Correction: Review charts to make sure everything is accounted for.
