Evidence:
Staff #2 was hired 2/14/2020 and the Out of State CPS form was not sent until July 10, 2020. The program closed 3/20/2020 and will reopen 7/27/2020 due to the National Health Crisis-COVID 19.Plan of Correction: Director will call the State to inquire about the background check status Starting 8/10/2020
