An unannounced inspection was conducted on 08/18/2023 based the center?s report of the school age building fire alarm malfunctioning after a scheduled fire evacuation drill on 08/07/2023. The preponderance of the evidence gathered during the investigation supports non-compliance with standards. A violation notice was issued. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 09/15/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 8VAC20-780-340-A Description: Based on interviews, the center failed to ensure the care, protection, and guidance of children.
1) Staff interviewed reported:
A) That the center implemented a monthly fire evacuation drill at approximately 11:30 a.m. on 08/07/2023. After the drill was completed, the fire alarm in the building serving the school age children would not silence.
B) The alarm continued to sound all day between approximately 11:30AM until approximately 5:00 p.m.
C) That the children enrolled in the school age program remained on site with staff at the center for the day while the alarm continued to sound for approximately five(5) and one half (1/2) hours.
D) That staff rotated the children between the outside playground and the inside of the building for lunch, snacks, water, and bathroom breaks with the alarm sounding between approximately 11:30 a.m. and 5:00 p.m.
E) That the fire alarm sound was extremely loud in the building.
2) A parent interviewed reported that that their child after extended exposure to the fire alarm is complaining of ear pain requiring medical assessment.
3) On 08/07/2023 between approximately 11:30AM and 5:00PM and while the alarm continued to sound, staff had the children outside in between the times they were in the building for lunch, snacks, water, and bathroom breaks. The outside temperature ranged between 85 and 92 degrees.Plan of Correction: Not available online. Contact Inspector for more information.
