Discussion of enrollment and forms. Discussion of background checks and the need for the Licensing Inspector to review them once they are obtained.
- Comments:

This monitoring inspection was initiated by licensing staff and concluded on 11/29/2023, between the hours of 10 am to 12 pm. There were 3 children present (10 points), ranging from infant to preschool-age, with 1 caregiver supervising. The children were engaged in the following: morning snack, art, games, freeplay, diaper changing, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 4 children records, 1 staff record, and 3 household records were reviewed.
Information gathered during the inspection determined that there were (6) non-compliances with the applicable standards, and code. These violations were documented on the Violation Notice issued to the home.

Standard #: 22.1-289.036-B-2 Description: Based on review, 1 of 3 household records indicated that the FDH does not have a copy of a fingerprint (check and provide personal descriptive information described in subdivision B 2 of ? 19.2-392.02) within 1 month of their 18th birthday.
Evidence -
1. On the date of inspection (11/29/2023), the fingerprint check on file for Household Member #2 was not available for review.
2. Based on information on file, the Provider had obtained an FBI check and did not follow the process as indicated on childcare.virginia.govPlan of Correction: Will send for background check.
Standard #: 22.1-289.036-B-3 Description: Based on review, 1 of 1 staff files; and 1 of 3 household records indicated that the FDH does not have an updated (within the past 5 years) copy of the results of a search of the central registry (CPS check) maintained pursuant to ? 63.2-1515 for any founded complaint of child abuse or neglect against him.
Evidence -
1. On the date of inspection (11/29/2023), the CPS check on file for Staff #1 was dated - 09/10/2018,
2. The CPS check on file for Household Member #1 was dated - 09/10/2018.Plan of Correction: Will send for background checks.
Standard #: 8VAC20-800-100-A Description: Based on review, 1 of 4 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.
Evidence -
1. On the date of inspection (11/29/2023), documentation of a physical examination was not available for review for Child #4.
2. The start date for Child #4 was listed as 06/13/2023. Child #4 was in attendance on the date of inspection.Plan of Correction: Will get all missing information and/or documents.
Standard #: 8VAC20-800-60-B Description: Based on review, 2 of 4 children records indicated that not all of the required information was contained within each child's file.
Evidence -
On the date of inspection (11/29/2023), the following documentation was not available for review:
1. The addresses for the two emergency contacts listed for Child #3.
2. The Authorization for Emergency Medical care (found on page 2 of the Child's Record) for Child #2*. This same information was cited during an inspection previously conducted on 05/03/2023.Plan of Correction: Will get all missing information and/or documents.
Standard #: 8VAC20-800-70-A Description: Based on review, 1 of 4 children files indicated that not all of the required documentation was contained within each child's file.
Evidence -
1. On the date of inspection (11/29/2023), page 2 of the Medication Administration - Decision to Administer form for Child #1 was not available for review. This page contains a signature by the parent that the provided information was reviewed.
2. Instead, on the back of page 1 was copied the Information for Parents form.Plan of Correction: Will get all missing information and/or documents.
Standard #: 8VAC20-800-90-C Description: Based on review, 2 of 4 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations (for a child who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.
Evidence -
1. On the date of inspection (11/29/2023) the following children did not have an updated immunization page on file: Child #2 (immunization page on file, dated - 04/24/23), and Child #3 (immunization page on file, dated - 05/02/23).
2. On the date of inspection, based on information on file, both children were under the age of two years.Plan of Correction: Will get all missing information and/or documents.
