Standard #: 8VAC20-780-130-E Description: REPEAT VIOLATION
Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: The record for Child 5 contained a documented start date of 04/13/2022. Child 5 was determined to be 17 months old. The record for Child 5 contained two immunization records dated 03/11/2022 and 04/11/2023; additional immunizations were not obtained every 6 months between 03/11/2022 and 04/11/2023.Plan of Correction: Contacted the parents and requested the information during the inspection. Will get the immunizations updated every 6 months.
Standard #: 8VAC20-780-140-A Description: Based on record review, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s attendance; or within 30 days after the first day of attendance.
Evidence:
1. The record for Child 1 contained a documented start date of 06/05/2023; there was not a physical examination in the record.
2. The record for Child 3 contained a documented start date of 05/23/2022; there was not a physical examination in the record.

Standard #: 8VAC20-780-160-A Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record for Staff 5 contained a documented hire date of 05/30/2023; the tuberculosis screening was dated 06/02/2023. The tuberculosis screening was not obtain prior to contact with children.Plan of Correction: Will get TB screenings prior to coming into contact with children.
Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 3 did not contain the following information: the phone number of child?s physician; name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete); and documentation of child updates and confirmation of up-to-date information in the child?s record.

The record for Child 5 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there were no addresses were not provided for the designated people).Plan of Correction: Contacted the parents and requested the information during the inspection.
Standard #: 8VAC20-780-70 Description: REPEAT VIOLATION
Based on record review and discussion with staff, the center failed to ensure that staff records contain all of the elements as required by the standards.
Evidence:
1. The record for Staff 1 did not contain documentation to demonstrate that the individual possesses the education, certification and experience required by the job position.
2. The record for Staff 5 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment (there was only one reference available for review).Plan of Correction: Will get the information into the records.
