Discussed with Head of School: *Sanitation inspection requirement - Fairfax County "User Guide" to be sent. *Age range on application - age cannot be lowered until equipment is in place for younger children.

Comments:

An unannounced, on-site renewal inspection was initiated on 09/16/24 and completed on same, as a part of the licensure period. The on-site inspection began at 2:05 pm and ended at 4:25 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 2 children?s records and 5 staff records on-site. This inspection included; document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/25/24. A POC submitted after this date will not appear on the public website.

Description:	Any individual required to undergo a background check in accordance with subsection A shall authorize the child day center to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 within 30 days from employment, The center did not have the results of a search of the central registry for Staff #5, who has been employed for 3 months. (Staff 5)

Description:	Repeat Violation Systemic Deficency An employee of a licensed child day program must be denied continued employment if the licensed child day program does not have a central registry finding within 30 days of employment. The center did not discontinued employment for Staff #5 when they did not have a completed central registry search on file for 3 months.

Description:	Cribs, cots and mats used by children during the designated rest period shall have linens consisting of a top and bottom cover. One of five children on mats during the designated rest period did not have a bottom cover in use on their mat.
