An unannounced monitoring inspection was initiated on 02/01/2024 and completed on 03/01/2024 in response to a self reported incident received by the department on 01/31/2024, related to supervision. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Standard #: 8VAC20-780-40-E Description: Based on interviews and a review of documentation, the licensee did not ensure that the center?s activities and services were maintained according to its own policies and procedures that are required by the standards.
Evidence:
The center?s procedures for identifying where attending children are at all times states that staff will ?monitor the exits?. However, on 1/31/2024 at approximately 12:35pm, Child A (3 years old) was able to leave the classroom through an open door, following Staff #1 down the hallway without Staff #1?s knowledge. The child then walked out of the unmonitored front exit when Staff #1 went into the office.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-A Description: Based on interviews the center failed to ensure that staff supervising children ensured their care, protection and guidance when a child was able to leave the center unaccounted for.
Evidence:
1. On 1/31/2024, Child A (3 years old) exited the center at approximately 12:35 pm at parent pick up without the awareness of staff at the center.
2. The child walked out of the front exit of the center to their parent arriving in the parking lot.
3. Staff were unaware of the child?s whereabouts for approximately 1 minute until the parent of the child returned to the classroom with the child.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-F Description: Based on interviews, the center did not ensure that children under the age of 10 years of age were not always kept within actual sight and sound supervision.
Evidence:
On 1/31/2024, staff did not have Child A (3 years old) within actual sight and sound supervision for approximately 1 minute from 12:35-12:36 pm when the child followed Staff #1 out of the open classroom door without Staff #1?s knowledge. The child walked down the hallway, and when Staff #1 walked into the office, the child exited the building through the front door into the parking lot where their parent had arrived to pick up the child.Plan of Correction: Not available online.
