This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.
A monitoring inspection was initiated on 8/30/2021 and concluded on 8/30/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 4 children present with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 household members and 1 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-170-A Description: Based on record review, the provider did not obtain from each adult household member a current Report of Tuberculosis Screening form documenting the absence of tuberculosis in a communicable form.
Evidence:
Household member #3 did not have documentation of a current report of Tuberculosis Screening form on file.Plan of Correction: I have an appointment September 17.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider did not obtain for each caregiver and each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
The records of Household member #1 and Household member #2 did not contain a current Report of Tuberculosis (TB) Screening form. The last screening form was obtained on 6/3/2019 and it expired on 6/3/2021.Plan of Correction: I have an appointment September 17.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review and staff interview, fingerprints were not obtained within 30 days of a person residing in the home becoming 18 years old.
Evidence:
Documentation of a fingerprint check was not obtained within 30 days of a person residing in the home becoming 18 years old. Household member #3 is over the age of 18. Staff stated the fingerprint check was not completed.Plan of Correction: I just received the fingerprints.
