An unannounced monitoring inspection, resulting from a self-reported incident, was conducted on-site on 05/10/2023 and concluded with telephone interviews on 05/16/2023. The inspector reviewed 1 child record and interviewed 4 staff. The inspector arrived at the center at 9:50 am and departed at 10:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-60-A Description: Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all of the elements as required by the standards.
Evidence: The record for Child 1 did not contain the following information: name and phone number of the child?s physician; and the first date of attendance.Plan of Correction: Will thoroughly review all children's records before they start.
