A monitoring inspection was conducted on 08/21/2023. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence:
The center's emergency map that was posted near the front door, did not include the shelter-in-place route.Plan of Correction: All maps and plans will be updated to include shelter-in-place routes and procedures.
