*Parent decision to remove medications for allergies as no longer needed. Physician should provide documentation to support this change.
*Balanced Mixed Age grouping requirements. Letter provided by the Head of School regarding Guidepost Montessori stance on "moving children up" from corporate counsel.

A monitoring inspection was conducted today per the conditions of a Provisional License.
Staff records, child records, medications and injury reports were reviewed. See Violation Notice for non-compliances cited.
Violations discussed with Head of School: Injury reports (10 reports reviewed), Staff records background search documentation, TB documentation, medications and allergy action plans.

Standard #: 8VAC20-770-60-B Description: Based on a review of records, one staff record did not contain documentation of a completed Sworn Disclosure Statement (SDS) prior to employment.
Evidence:
1 of 3 staff records reviewed was missing a completed SDS prior to employment. (Staff A: Date of Hire (DOH): 4/3/23).Plan of Correction: The employee has a Sworn Disclosure Statement signed.
Standard #: 8VAC20-780-160-A Description: Based on a review of records, one staff record does not contain documentation of a negative Tuberculosis(TB) screening submitted at the time of employment and completed within the past 30 calendar days.
Evidence:
1 of 3 staff records reviewed contained a TB negative result that was dated 8/31/21. Staff date of hire: 4/3/23. (Staff A).Plan of Correction: Staff A had a chest x-ray on 8/31/2021 and will obtain a doctor's note verifying she does not need to have another x-ray done at this time.

Evidence:
The parent handbook states: "It is the parent's responsibility to keep a record at home to maintain medications at the school that are up -to-date." One Epi-pen at the center expired 1/2023. (Child 1)Plan of Correction: The parent will provide a new Epi-pen.
