Comments:
An unannounced monitoring inspection was conducted today.
All rooms observed were in ratio during the visit.
2 staff and 2 child records were reviewed.
Medications, injury reports, emergency evacuation drills, emergency supplies, first aid supplies, policies and procedures and cot spacing for rest time were reviewed.
See violation notice for non-compliances cited today.

Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the center failed to discontinue employment of staff when Central
Registry Search results were not obtained within 30 days of employment.
Evidence:
Staff A did not have a completed search of Virginia Central Registry search or
documentation of follow-up within 30 days of employment. (Date of Hire: 9/7/23)

Plan of Correction: Staff A has resubmitted the Central Registry Paperwork. Central registry letter with no match is present in the staff file.

Standard #: 8VAC20-780-160-C
Description: Based on a review of records, one staff record did not contain documentation of the results of a follow up tuberculosis (TB) screening at least every two years.
Evidence:
Staff B's record contained a negative TB screening result dated.4/16/22. (expired)

Based on a review of records, one child record does not contain all required information,

Documentation of having viewed Proof of Birth documentation. (Child 1)

Plan of Correction: The record has been updated and the child's proof of identity document has been added to the student record.

Standard #: 8VAC20-780-60-A-8
Description: Based on a review of records, child records did not contain a written care plan to include instructions from a physician regarding a food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
Child #2's allergy action plan cannot be followed as a listed medication is not on site at the facility.

Plan of Correction: Child #2 does not have a food allergy. Child #2 has a special diet restriction as listed on the child's VA School form written by the child's physician. No allergy action plan or medication is needed.

Standard #: 8VAC20-780-80-A
Description: Based on staff interviews, the center did not maintain written attendance for each group of children that documented arrival and departure as it occurred.
Evidence:
1. No written attendance was maintained with the group of children going to the Gross Motor Room at 12:30pm on 4/12/24.
2. On 4/12/24 no written attendance was maintained with each group of children for the Children's House 4 room when the group split to go back to the classroom. Staff B took some children back to the room, while Staff A stayed in the gross motor room with children preparing to go to the classroom. Staff stated they knew the number of children that left; however, no name to face check was conducted to document which children left, and which children were still in the room.

Plan of Correction: Staff are completing the West Alex Roll Call sheet which includes:
*A 30 minute head count tracker
*Name to face tracker
*Guide Sign-in and out.
In addition all children are signed in to the center by their parents/pickup person and by the guide who receives them and the guide who releases them to their parent/pick up person.

Standard #: 8VAC20-780-350-B-5
Description: Based on interviews with staff, a ratio of 1 staff to 18 children was not maintained for a group of children that contained 5 year old children.
Evidence:
On 4/12/24 - 1 staff person took 21 children to the Gross Motor room on the first floor of the building at approximately 12:30PM.

Plan of Correction: Based on attendance sheet records pulled for that day, 19 5-year-olds were in attendance on 4/12/24. There is not record of the exact number of children taken to the GMR that day which is stated in the previous violation.

Systemic Violation
Based on a review of medications, one medication does not have an authorization available for review.
Evidence:
There is no authorization available for an Epi-Pen at the center. (Child 2)

Plan of Correction: Child #2 does not have a food allergy. Child #2 has a special diet restriction as listed on the child's VA School form written by the child's physician. No allergy action plan is needed.

Standard #: 8VAC20-780-510-I
Description: Repeat Violation
Based on a review of medications, one medication present at the center was not maintained in the original, labeled container.
Evidence:
An Epi-pen at the center did not have a prescription label attached to the medication.

Plan of Correction: The box in question contained only a trainer Epi-pen. Child #2 does not require an Epi-pen. the Trainer has been removed from the room.
