This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 09/21/2020 and concluded on 09/23/2020. The director was contacted by telephone to initiate the inspection. There were 15 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 4 children's records and 2 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the director COVID-19 Essential Guidance for Child Care Programs.

Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center failed to ensure that children's records contain all of the required elements.
Evidence:
1. The enrollment record for child 1 lacked the following required information:
a. Name, home address, and home phone number for one parent;
b. The work phone number and place of employment for each parent who has custody;
c. Documentation of viewing proof of identity and age.
2. The enrollment record for child 2 lacked the following required information:
a. The work phone number and place of employment for each parent who has custody;
b. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
3. The enrollment record for child 3 lacked the following required information:
a. The work phone number and place of employment for each parent who has custody;
b. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: Each parent will be asked to provide current information regarding employment and emergency contacts.
Standard #: 22VAC40-185-90--A Description: Based on record review and interview, the center failed to ensure that the following statement is included in the written parent agreement that is required to be in each child's record before the first day of attendance:
*A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Evidence:
1. The written communicable disease statement in the record for child 1, child 2, child 3 and child 4 is incomplete and does not include all required information.
a. The statement on the enrollment forms reads as follows:
"I agree to inform the YMCA child care staff/director within 24 hours or the next business day by the State Board of Health, except for life threatening diseases which must be reported immediately."
2. The statement lacks the following required information:
"....after his child or any member of the immediate household has developed any reportable communicable disease...."Plan of Correction: The parent agreement part of the enrollment form will be updated to include the complete communicable disease statement.
