An unannounced inspection was conducted on September 26, 2023 in response to a complaint regarding injury reporting. Interviews with staff were completed and documentation was reviewed.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.

Standard #: 8VAC20-780-550-P Complaint related: No Description: Based on interview and record review, the facility failed to maintain a written record of children's minor injuries on the day of the occurrence. Evidence: On 8/8/2023 and 8/22/2023, the facility notified the parent that the child had a scratch, but did not complete an injury report. Plan of Correction: The facility will document injuries on an injury report the day it occurs.
