Discussed areas within the Infant classroom and consideration for future replacement of cribs. Keep all hazardous materials locked during operating hours. Only utilize play sand approved for children in sensory tables.

An unannounced inspection was initiated and completed on October 25, 2024, in response to a complaint received by the licensing office on October 24, 2024, relating to allegations of special services provided. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 4, 2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Complaint related: No Description: The center shall ensure that areas and equipment of the center inside are maintained in a clean, safe and operable condition.
-There were two pieces of infant climbing equipment which had tears and frayed areas.
-Areas of peeling paint were found in the two year old classroom by the emergency exit.Plan of Correction: The pieces of climbing equipment were removed and the paint will be fixed.
Standard #: 8VAC20-780-280-D Complaint related: No Description: Cleaning and sanitizing materials shall be stored in areas physically separate from food.
Two bottles of cleaning materials including a Pinesol and Water mixture were stored in a cabinet with children?s food pouches in the Toddler classroom.Plan of Correction: The cleaning supplies will not be stored in areas containing food moving forward.
Standard #: 8VAC20-780-340-D Complaint related: No Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.
Staff #2 and Staff #3, who do not meet lead teacher qualifications, were alone with a group of Infant children and stated they have been working without a lead teacher. The director stated that they are checked on approximately every 45 minutes throughout the day.Plan of Correction: A current staff member is in training and finishing up to be assigned as the lead teacher for the classroom.
Standard #: 8VAC20-780-420-E-1 Complaint related: No Description: The center is required to post a daily record which can be easily accessed by both the parent and the staff working for each infant and contain information regarding: a. The amount of time the infant slept; b. The amount of food consumed and the time; c. A description and time of bowel movements; d. Developmental milestones; and e. For infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.
The center does not keep daily records of the required information for each infant.Plan of Correction: Paper daily reports were printed out and will be used for infants moving forward.
Standard #: 8VAC20-780-570-A Complaint related: Yes Description: The center is required to use high chairs, infant carrier seats, or feeding tables for children under 12 months who are not held while being fed.
Child #4, 8 months old, was placed directly on the floor to drink a bottle themselves.Plan of Correction: Staff discussed the requirement and we will ensure infants are fed properly moving forward.
