Comments:

An unannounced monitoring visit was conducted today. Three children were present with the provider for six points which is within ratios for supervision. The provider also does both before and after school care for elementary age children. The basement has been updated and has a play area, eating area, small kitchen to prepare meals and restroom. Observed children returning from the park, hand washing, record review, emergency preparation and training for the provider. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on record review and provider statement, a child did not have a physical exam within 30 days of attendance. Evidence: Child #1 enrolled 2/27/23 and no physical was in the record.
Plan of Correction:	The parent has made an appointment to obtain a physical and will bring a copy to the provider as soon as it is obtained.
Standard #:	8VAC20-800-70-B
Description:	Based on record review, the provider did not obtain the parent's signed acknowledgement on the Information For Parents form. Evidence: Child #1's information for Parents form does not have the parent signature acknowledging receipt of the information.
