Licensing standards regarding staff records and lead water testing in 2025 were discussed with the director.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, renewal on-site monitoring inspection was initiated on July 19, 2024 and completed on July 19, 2024. The on-site inspection began at 10:05pm and ended at 12:02pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 6 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation and interview, the center did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
Evidence:
1. The three year old room had a pair of sharp and pointy adult scissors on top of a toy storage shelf accessible to children.
2. The director, staff #1 confirmed that the scissors were sharp and accessible to children.Plan of Correction: Remove all adult scissors out of every room.
Standard #: 8VAC20-780-280-E Description: Based on observation and interview, the center did not ensure that cleaning materials and insecticides/pesticides be stored in areas physically separate from each other.
Evidence:
1. A bottle of insect repellent belonging to child #2 was stored in a cabinet next to a spray bottle of bleach water that was used for cleaning and sanitizing the classroom.
2. The director, staff #1 confirmed that the insecticide and cleaning material were not stored separately.Plan of Correction: The director will store any bug spray in the office in a locked cabinet, away
from any other chemicals.
Standard #: 8VAC20-780-520-C Description: Based on observation, interview and record review, the center did not ensure that if diaper ointment or cream is used, the required information be obtained.
Evidence:
1. A tube of diaper ointment was applied to child #1 earlier in the week and the record did not contain parent written authorization documentation noting any adverse reactions.
2. The diaper ointment tube for child #1 did not contain a label with the child?s name.
3. A record of diaper ointment use for child #1 was not maintained to include child's name, date of use, frequency of application and any adverse reactions.
4. The director, staff #1 confirmed that all required information for diaper ointment was not obtained before applying to child #1.Plan of Correction: The director will print out forms for the parents to fill out for diaper cream. Once the parents fill out the form it will be stored in the child's record. The teacher in the room will fill out the paperwork daily if the cream is needed daily.
Standard #: 8VAC20-780-520-D Description: Based on observation and interview, the center did not ensure that if insect repellent is used, the required information be obtained.
Evidence:
1. The insect repellent for belonging to child #2 did not contain written parent authorization noting any adverse reactions to the insect repellent.
2. The director, staff #1 confirmed that the insect repellent did not contain written parent authorization of the required information.Plan of Correction: The parents will be required to fill out information on any over the counter spray
for the children.
