The following documents were email to the Family Day Home (FDH): a blank Policy for the Administration of Medications form, and page 12 of the FDH Renewal Application (Business Entity - LLC).
- Comments:

The following on-site renewal inspection took place between the hours of 10:30 am through 11:30 am, on 12/10/2021. The Licensing Inspector (LI) met with the Provider on site at the Family Day Home (FDH) located in Arlington, Virginia. At the start of this unannounced renewal inspection there were a total of 4 children observed (12 points) with 2 caregivers. A total of 5 children?s files, 3 staff files, and 1 household member file was reviewed. The LI reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no meds on site); special care and emergencies; and nutrition.
Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-830-C Description: Based on observation, the documentation of past emergency evacuation drills (fire drills) were not documented.
Evidence -
1. On the date of inspection (12/10/2021), fire drills for the months of October, November, and December 2021 were not documented on the posted drill log.
2. The Provider confirmed on the date of inspection that these drills were conducted, but were not recorded on the posted drill log.Plan of Correction: The drill log will be updated to include missing drills.
Standard #: 8VAC20-800-60-B Description: Based on review and interview, 1 of 5 children files indicated that not all of the required documentation was contained within each child's file.
Evidence -
1. On the date of inspection (12/10/2021), the Policy for the Administration of Medications form was not available for review for Child # 3.
2. The Provider stated (during an interview conducted on the date of inspection) that all forms were given to the parent of Child #3 to complete and has yet to be returned.Plan of Correction: I will check with the parent of Child #3 to obtain this form. If needed, a blank form will be completed and returned.
