Comments:

An unannounced, on-site renewal inspection was initiated and completed on November 18, 2024. The on-site inspection began at 2:25 pm and ended at 3:40 pm. The inspector reviewed compliance in the areas listed above. There were no children present and 1 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included observations, document review, interviews, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 26, 2024. A POC submitted after this date will not appear on the public website.

Description:	(Repeat Violation) The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for Child #5, who had been attending for one month, did not have documentation of an immunization record on file.

Description:	(Repeat Violation) Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. 1. The record for Child #1, and child #3 enrolled for one month, was missing the following required components: the address for 1 of 2 emergency contacts. 2. The record for Child #4, and child #5 enrolled for one month, was missing the following required components: the address for 2 emergency contacts and the child?s doctors name and contact number.

Description:	Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. The record for Staff #1 and Staff #3, did not contain documentation of 16 hours of annual training in 2023.
