Technical Assistance:

1. Reviewed fan requirement and related safety requirements when the temperature in the home exceeds 80 degrees. It was reported that the home AC stopped working this morning and at the time of the inspection the daycare areas of the home were measured to be 81.8 degrees. Areas were humid, and one fan had been placed in the childcare area. Assistant #2 reported that a repair call had been placed. 2. Discussed the importance of ensuring records are maintained in a manner that are always accessible. 3. Inspector agreed to provide model forms for the Information for Parents, and emergency drill record. 4. Staff:child ratios are expected to be maintained at all times. When a caregiver leaves the direct care and supervision, they are leaving considered to be leaving the ratios.

Comments:

An unannounced renewal inspection was conducted at 12:57pm. There were 7 children = 17 points with the provider + assistant #2 upon arrival. Assistant #1 arrived to the home just after the inspector arrived. Children were napping during the inspection. Safe sleep spaces were not provided for all children today. The childcare areas located on the lower level of the home were clean and well supplied with toys and equipment for the children. The outdoor play area located in the backyard offered many play options. Each child packs their own food daily. Areas of non-compliance were found with regulations and laws reviewed. Description:	Based on records reviewed and interview conducted, it was determined that 2 of 3 children?s records did not include required information. Evidence: 1. Child #8 ? The record did not include the following information: parent home + work addresses + phone numbers; emergency contact addresses + phone numbers; individuals authorized to pick-up the child; medical insurance information; written authorization for emergency medical care; documentation of parent receiving written policy information, emergency plan, and liability insurance; and verification of proof of identity. Assistant #2, in the presence of the provider, stated the documents were scanned into the computer, but lost when the computer recently crashed. 2. Child #9 ? The record did not include the following information: medical insurance policy number; documentation of parent receiving written policy information; and verification of proof of identity. Description:	Based on records reviewed and interview conducted, it was determined that a record of emergency drills has not been maintained. Evidence: The last recorded drill documented was 12/5/21. Assistant #2, present with the provider, indicated the drills were completed, but was not able to provide said record.

Standard #:	8VAC20-800-100-A
Description:	Based on records reviewed, it was determined that 2 of 3 children?s records did not include a physical exam within the first 30-days of attendance. Evidence: 1. Child #4 ? There was no physical exam on-file. The child began attending in Se ptember 2021. 2. Child #9 ? There was no physical exam on-file. The child began attending in March 2021.

Standard #:	8VAC20-800-280
Description:	Based on observations made, it was determined that a potentially poisonous substance was accessible to children. Evidence A can of spray air freshener product was on a ledge between the kitchen area + main daycare area and within reach of children in care. At the time of observation Child #1, a 5 year old, was standing beside the ledge and could have reached the product with ease.
Plan of Correction:	Removed can-placed in safe location out of reach of children.
Standard #:	8VAC20-800-500-C
Description:	Based on observations made, it was determined that stationary outdoor play equipment did not have a minimum of 6? perimeter from other play structures or obstacles. Evidence: 1. 2 plastic slides were located within inches of one another and shared a fall zone perimeter. 2. 2 larger climbing/slide structures were within inches of the wooden yard fence.
Plan of Correction:	Moved structures to ensure 6' distancing.

Description:	Based on observations made and interview conducted, it was determined that a small play trampoline was placed in the main daycare area. Evidence: The provider stated that it was there for a 5 year-old child to play.

Description:	Based on observations made and interview conducted, it was determined that there was not a designated crib for a child under the age of 12 months old. Evidence: Child #8, a 10-month old, was observed napping in a swing and there was no crib available to the child. The provider stated the parent requested the child nap in the swing and that all cribs were being used by other children in care.

Description:	Based on observations made, it was determined that a mesh-sided portable crib was being used as a sleep space for one child. Evidence: Child #9 was observed napping in the mesh-sided crib.
