Technical Assistance:

1. Discussed not storing/setting items on the top of the diaper changing pad to reduce the spread of germs. 2. Reviewed the requirement to NOT post the Supplemental Information page of the inspection report, as it contains confidential information. 3. The inspector will send training information, background check links, and a model form discussed.

Comments:

An unannounced monitoring inspection was conducted at 10:15am. The primary focus of this inspection was to determine compliance with violations cited in the 2/7/24 inspection. Observed 10 children totaling 22 points with 2 caregivers. Ratios and supervision were in compliance. Children were observed playing with toys, listening/singing to music and having active outdoor play. The caregivers were actively engaged with the children and activities. The daycare areas of the home were found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Description:	Repeat violation. Based on records reviewed, 1 of 3 caregivers did not complete a TB test/screening every 2 years. Evidence: The TB test on-file for Caregiver #3 was dated 3/25/22.
Plan of Correction:	TB for the 1 assistant has been requested and appt is later this month. gave assitant until 6/30/24 [sic]
Standard #:	8VAC20-800-100-A
Description:	Repeat violation. Based on records reviewed, 2 of 14 children's records did not include documentation of a physical exam within the first 30-days of attendance. Evidence: 1. Child #8-No physical exam on-file. Child enrolled January 2023. 2. Child #10-No physical exam on-file. Child enrolled March 2022.
Plan of Correction:	1. Child #8 - we located her physical - folder on computer and added to hard copy binder (see attached doc) resolved 6/4/24 2. child #10-requested copy of latest physical to add to binder. They did provide one in march but it was a dual copy of her brother who also attends the daycare. gave parent till 12/19/24 [sic]

Description:	Repeat violation. Based on records reviewed, a written record of children in attendance each day has not been maintained. Evidence: There were no attendance records for 5/17/24-6/2/24.
Plan of Correction:	we are seeking a table to place near our entrance door that will hold the sign-in & sign-out book. taking attendance daily - will buy table by 6/30/24.

Description:	Repeat violation. Based on records reviewed, 2 of 14 children's records did not include documentation of the child's proof of identity verification within the first 7-days of attendance. Evidence: 1. Child #10-There was no proof of identity documentation. Child enrolled March 2022. 2. Child #16-There was no proof of identity documentation. Child enrolled February 2022.

Standard #:	8VAC20-800-90-C
Description:	Repeat violation. Based on records reviewed, 1 of 14 children's records did not include documentation of immunizations every 6 months until the age of 2 years old. Evidence: Child #13-The immunizations on-file were dated August 2023, when the child was 12 months old.
Plan of Correction:	No issue - in child #14 binder there is immunizations dated 11/8/23 (see also attached) - start date was Sept 2023 No Issue-The next immunization is due in July 2024. [sic]
