An unannounced monitoring inspection was conducted on March 13, 2024, from 9:30am-10:40am. At the time of entrance, ( 2 ) children ( 6 ) points were in care with (1 ) caregiver present. The inspector reviewed 3 children?s records and 1 caregiver record. Children were observed during snack time, and playing with toys. No medications were on site. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Standard #: 8VAC20-800-100-A Description: Based on a review of children records, the provider has not obtained documentation of the child's physical examination within 30 days of first day of attendance.
Evidence: There is no documentation of child # 1 and child #3 physical examination, both first day of attendance 9/11/2023.Plan of Correction: I will get the information from the parents.
Standard #: 8VAC20-800-70-A Description: Repeat Violation:
Based on a review of children records, the provider has not provided in writing whether or not there is liability insurance to include the child's name, parents signature and date obtained before the child's first day of attendance.
Evidence: The information about insurance liability has not been obtained from parents of child # and child #2, both first day of attendance 9/11/2023.Plan of Correction: I will get the information from the parents.

Based on a review of children records, the provider has not obtained documentation that the child has been adequately immunized prior to attending the family day home.
Evidence: Child # 2, first day of attendance 9/11/2023 does not have documentation of a immunization record.Plan of Correction: I will get the information rom the parents.
Standard #: 8VAC20-800-140-B-3 Description: Based on a review of provider record and provider interview, the provider has not maintained a current certification in CPR.
Evidence: The provider has not updated the CPR certification. The CPR certification expired 1/6/2024.Plan of Correction: I will get my certification.

Standard #: 8VAC20-800-240-A Description: Based on observation, the bathroom is not maintained in a clean and operable condition.
Evidence: There was peeling pint observed in the hallway bathroom. The paint was peeling from the left side wall and above the toilet. There was a purple-black coloring covering the ceiling of the shower to indicate presence of mold.Plan of Correction: I will contact my landlord to fix the bathroom walls.
