An announced initial inspection was conducted today to determine compliance with licensing standards. The inspection was initiated at 10:30am and concluded at 12:00pm. There were 4 children directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, the physical plant, cribs, emergency supplies, and policies were inspected. Required documents were posted and the provider's record was reviewed. Children were observed singing in English and Spanish, reviewing the days of the week and reading books. The home was well supplied with toys and equipment for the children. The backyard offers outdoor play space for the children. Areas of non-compliance are identified on the Violation Notice.
A recommendation will be made for the issuance of a six month Conditional License. Questions about this inspection may be directed to maria.soto@doe.virginia.gov.

Standard #: 8VAC20-800-710 Description: Based on review of medications, prescription medication was not maintained in the original, labeled container.
Evidence:
The prescription medication for Child #3 was not in the original container with a prescription labeled attached.Plan of Correction: I immediately finished the inspection, contacted the parents of Child #3 and informed them that I need the prescription in its original box, they brought it in in the afternoon.
Standard #: 8VAC20-800-750-A Description: Based on observation, not all topical skin products were labeled with the child's name.
Evidence:
There were two diaper ointment/creams in the diaper station that were not labeled with a child's name.Plan of Correction: At the time of the children's nap, labeled all the creams with their respective names.
Standard #: 8VAC20-800-70-A Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance.
Evidence:
The record for Child #1, #2, #3, #4 and #5 were missing the Liability Insurance Declaration form and the Information for Parents form.Plan of Correction: At the end of my day, I gave the parents the missing documentation and they returned it to me the next day.
