Comments:
An unannounced monitoring inspection was conducted on July 10, 2023 from 10:00 a.m. through 12:00 p.m. under the supervision of the Licensing Administrator. Upon arrival, there were 35 children, ages 2 through 7, in the care of 7 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-770-40-D-6
Description: Based on record review the center accepted a criminal history check for 1 staff member that was dated more than 90 days prior to the date of employment.

Evidence:

1. Five staff records were reviewed.
2. Documentation of fingerprint checks for Staff #1 was dated 02/21/2023.
3. Staff #1's date of hire was documented 06/26/2023.
4. The director confirmed the fingerprints had been obtained 90 days prior to Staff #1's first day of employment.

Description: Based on record review the center failed to obtain emergency contact addresses for three staff.

1. Five staff records were reviewed.
2. Staff # 1, Staff #2 and Staff #3 did not have documentation of emergency contacts addresses.
3. The director confirmed there was no documentation of addresses for emergency contacts.

Description: Based on medication review, the center failed to have two medications labeled with the child's name, the name of the medication, the dosage amount and the times or times to be given.

Evidence:

1. The medication and medication paperwork was reviewed for one child.
2. One Auvi-Q emergency medicationwas not labeled with the prescription label attached to include the child's name, name of medication, the dosage amount and time/times to be given.
3. One bottle of Children's Benadryl was not labeled with the child's name, dosage amount, and time/times to be given.
4. The director confirmed the medication was not labeled.

Description: Based on observation the center failed to have two bottles of sunscreen inaccessible to the children in care.

Evidence:

1. Two bottles of sunscreen were observed in the Two-year old classroom in cubbies that were accessible to the children in care.
2. The classroom teacher immediately removed the two bottles of sunscreen and placed them in a locked cabinet.
