8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

An unannounced, on-site Subsidy Health and Safety (SHSI) annual inspection was initiated and conducted on August 27, 2024 to review supplemental health and safety requirements. The on-site inspection began at 10:10 AM and ended at 12:01 PM. The inspector reviewed compliance in the areas listed above. There were four (4) children present and one (1) caregiver. The inspector reviewed five (5) children's records, two (2) household members' records, and one (1) caregiver record on-site. This inspection included document review, tour of the home, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-790-190-B Description: Based on record review, the child's information did not include name, address and telephone number for each custodial parent's place of employment.
Evidence:
1. The record for Child #1 did not have the name, address, and telephone number for each custodial parent's place of employment.
2. The record for Child #3 did not have the name, address and telephone number for each custodial parent's place of employment.
3. The record for Child #4 did not have the name, address and telephone number for each custodial parent's place of employment.Plan of Correction: I will speak to the parents again and explain I must have the name, address and telephone number of their place of employment.
Standard #: 8VAC20-790-390-A-1 Description: Based on observation, the children's hands were not washed with soap and running water or disposable wipes before eating snacks.
Evidence: Four children did not have their hands washed or wiped with a disposable wipe prior to eating their morning snack.Plan of Correction: I usually wash their hands before eating, but I forgot today. I will wash their hands before eating.
Standard #: 8VAC20-790-390-A-3 Description: Based on observation, the caregiver did not wash their hands with soap and running water before and after helping a child use the toilet or changing a diaper.
Evidence: The caregiver did not wash their hands with soap and running water after diapering Child #1 and Child #4.Plan of Correction: I wash my hands when I washed the child's hands but I will start washing my hands after I clean the diapering surface.
