A mandated monitoring inspection was conducted on December 11, 2023 from 11:30 A.M.-1:30 P.M. There were six children present for a total of 18 points, ranging in ages from five months to four years of age, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and two caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-750-B Description: Based on observation, the provider failed to ensure that diapering ointment shall be inaccessible to children.
Evidence:
In the room with cubbies and diaper changing table there was a tube of Desitin Ointment on the floor next to the cubbies. Children have access to this room and area at all times.Plan of Correction: The diaper ointment most likely fell out of a diaper bag. The provider will check diaper bags and pull out any diaper ointments so they are not accessible to children.
Standard #: 8VAC20-800-100-A Description: Based on review of children records, the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4's date of enrollment was 11/01/2023. There was not documentation in the record of a physical examination.
2. The provider verified that there was not a documentation of a physical exam in the record.Plan of Correction: The provider will follow-up with the parent to obtain the physical examination.
Standard #: 8VAC20-800-60-A Description: Based on a review of children records, the provider failed to obtain all required information for children records.
Evidence:
1. Child #1 and #4's file did not contain the date of enrollment.
2. Child #2's file did not contain the name and policy number of the child's medical insurance
3. Child #4's file did not contain a complete address for the designated persons to contact in case of an emergency if the parent cannot be reached.Plan of Correction: The provider will contact all parents to get the missing information and will audit additional files to ensure all information is complete.
Standard #: 8VAC20-800-280 Description: Based on observation, the provider failed to ensure that hazardous materials were inaccessible to children.
Evidence:
1. In the sleeping area on a shelf next to the diaper changing table there was a can of Microban Sanitizing Spray, and a container of Clorox Wipes.
2. In the bathroom there was a bottle of Mrs. Meyers Toilet Bowl Cleaner on the floor next to the toilet.
3. Manufacturer label stated "keep out of the reach of children" and "caution".Plan of Correction: All areas that children use will be checked daily each morning to ensure that there are not hazardous materials accessible to children.
