Comments:

An unannounced focused inspection was conducted 9/21/2023 from 12:35pm through 1:55pm. There were 46 children directly supervised by 7 staff and all classrooms were in ratio. The physical plant, outdoor playground area, menu, 2 staff records, attendance, and diapering procedures were inspected. Children were observed napping and listening to story time and infant care was also observed during the inspection. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 10/4/2023. Description:	REPEAT VIOLATION Based on record review, the center did not obtain documentation of a negative tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children. Evidence: Staff #1 (Date of Hire: 9/19/2023) was present at the time of inspection and did not have documentation of a negative TB screening on file.

Description:	REPEAT VIOLATION Based on record review, the center did not obtain documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Staff #1 (Date of Hire: 9/19/2023) was present at the time of inspection and did not have reference checks on file.

Description:	REPEAT VIOLATION Based on observation, not all areas inside and outside the center were maintained in a clean and safe condition. Evidence: 1. The lower siding on the exterior of the school age building was cracked exposing sharp edges in the areas by the door leading to the outdoor play area and on the other side of the building that is within the outdoor play area. 2. There was a hole in the wall in the school age building by the door that leads to the outdoor playground area.

Description:	REPEAT VIOLATION Based on observation, the center did not ensure that where playground equipment is provided that there is adequate resilient surfacing in fall zones. Evidence: 1. On the Toddlers and Twos playground there was a spinning riding toy with four seats that had no resilient surfacing underneath in the fall zone. In addition, there was only three inches of resilient surfacing at the base of the two slides on the Toddlers and Twos playground that measured three feet and two feet in height. Six inches of resilient surfacing is required. 2. On the Threes and Pre-K playground there was only three inches of resilient surfacing at the base of the three slides on the composite play structure.
