Comments:
The inspection findings have been updated as a result of a first step review completed on 8/29/2024.

An unannounced, on-site monitoring inspection was initiated and completed on 6/6/2024, as a part of pending enforcement action. The on-site inspection began at 12:50pm and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 8 staff. The inspector reviewed 1 child's record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/2/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4
Description: REPEAT VIOLATION

Based on record review and staff interview, the center did not request a search of the child abuse and neglect central registry within 30 days of employment from each state where staff have lived within the last 5 years.

Evidence: Staff #1 (Date of Hire: 12/5/2023) has resided in North Carolina within the last 5 years and the center had not requested search results from the North Carolina child abuse and neglect central registry for Staff #5 as of the date of inspection, 6/6/2024.

Standard #: 8VAC20-780-160-A
Description: REPEAT VIOLATION

Based on record review, the center did not obtain documentation of a negative tuberculosis screening from each staff prior to employment that is signed by a physician, physician's designee, or an official of the local health department.

Evidence: Staff #2 (Date of Hire: 5/23/2024) had a tuberculosis screening result page on file but it was not signed by a physician, physician's designee, or an official of the local health department.

Description: Based on observation and interview, the center did not ensure that mats used by children during the designated rest/quiet period had linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

1. Three of eight children napping during the designated rest/quiet period in the Two's room did not have bottom covers on their mats.

Description: Based on observation, record review, and staff interview, the center did not ensure that all diaper cream and ointment requirements were met.

Evidence: In the Infant room, Child #1 had a diaper ointment on site and there was no accompanying written parental authorization noting any adverse reactions for the ointment.
