Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 8/28/2024. The on-site inspection began at 9:45am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/4/2024. A POC submitted after this date will not appear on the public website.

Description:	Based on record review, the center did not request the results of a Virginia Child Abuse and Neglect Central Registry Search for all staff prior to hire. Evidence: Staff #4 (Date of Hire: 8/26/2024) did not have documentation on file that the Virginia Child Abuse and Neglect Central Registry Search was requested prior to hire.

Description:	REPEAT VIOLATION Based on record review, the center did not obtain documentation of a negative tuberculosis screening from each staff prior to employment. Evidence: Staff #4 (Date of Hire: 8/26/2024) did not have documentation of a negative tuberculosis screening on file.

Description:	REPEAT VIOLATION Based on observation of the playground, the center did not ensure areas outside the center were maintained in a safe and operable condition. Evidence: On the toddler playground, the tot treehouse structure had a section of rubber siding the has deteriorated, cracked, and has pieces of rubber missing. Four wasps were observed inside the hole created from the missing rubber pieces.

Description:	REPEAT VIOLATION Based on record review and staff interview, the center did not ensure in each grouping of children there is at least one staff member who meets the qualifications of a program leader. Evidence: Staff #2 was providing direct care and supervision of the infant classroom at the time of inspection and was identified as the lead teacher in the infant room. Staff #2 did not have program leader qualification documentation on file at the time of inspection.
