An unannounced, on-site focused inspection was initiated and completed on 9/26/2024. The Department issued a Final Order to Deny Renewal Application for a License on 6/24/2024 as a result of high-risk violations. The on-site inspection began at 10:15am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/7/2024. A POC submitted after this date will not appear on the public website.

Areas and equipment outside the center shall be maintained in a clean and safe condition.
Wood trim around the door coming from the Pre-K room out to the Pre-K playground and the wood trim around the door and siding on the shed located in the Pre-K playground had several areas of peeling paint. Sections of the peeling paint was within reach of children and chipped paint was observed to have fallen on the ground nearby.Plan of Correction: Not available online.
