An unannounced monitoring inspection was conducted on June 5, 2024. There were no children in care. The center is closed for the summer and plans to reopen in September. The inspector reviewed compliance in the areas of administration, physical plant, special care and emergencies. A total of 5 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-60-A Description: (Repeat Violation) Based on a review of five child records, the center failed to ensure that each child enrolled has a record including all required information.
Evidence:
1. The record for child # 2 was missing the parents work number, the doctors phone number, and the address for the second emergency contact. Child #2?s enrollment date was 3/25/2024.
2. The record for child # 3 was missing the parent?s work number, the doctors contact number, and the address for two emergency contacts listed. Child #3 enrollment date was 8/14/2023.
3. The record for child # 4 was missing the doctors phone number. Child #4?s enrollment date was 2/5/2024.

Standard #: 8VAC20-780-550-F Description: Based on record review and interview, the center failed to ensure that lockdown procedures shall be practiced at least annually.
Evidence:
1. The center did not have documentation to demonstrate that a lockdown drill was conducted between January 2023 ? May 2024.
