An unannounced non mandated inspection was conducted on 09/13/2023. There were 55 children, ages 2 months-5 years, and 12 staff members present during the inspection. The inspector observed the following: free choice activities, large motor play outside, lunch and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 10:25 AM and departed at 12 PM.

Standard #: 8VAC20-780-70 Description: Based on review of ten staff records, the center failed to ensure that all records were complete per the standard.
Evidence:
Staff # 1 had a hire date of 08/14/2023 and did not have the two required reference checks prior to employment.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-240-B Description: Based on review of six staff records, the center failed to ensure that staff orientations were completed within seven days of hire.
Evidence:
Staff # 3 was hired on 07/3/2023. Staff # 4 was hired on 08/24/2023. These staff records reviewed lacked documentation that orientation had been completed. The model form was placed in each staff person?s record and but lacked signatures and dates to show if or when it was completed.Plan of Correction: Not available online. Standard #: 8VAC20-780-280-B Description: Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.
Evidence:
In the bathroom of the two-year-old room, the cabinet under the sink had several containers of chemicals there were unlocked. Children were observed in the room and the cabinet was accessible to the children.Plan of Correction: The bathroom door was shut and locked during the inspection.
Standard #: 8VAC20-780-290-A-3 Description: Based on observation, the center failed to
ensure that all electrical outlets had protective coverings.
Evidence:
1. There was an electrical outlet in the infant room that did not have a protective covering. It was in by the infant swing observed being used by child in care.
2. There were two electrical outlets in the toddler room that did not have a protective covering. Observed one child continually taking out the protective covering once the staff replaced it. The director moved the bookshelf in front of the electrical outlet to ensure that children were not able to access.Plan of Correction: The director will purchase new coverings to ensure children care not take them out.
Standard #: 8VAC20-780-340-D Description: Based on observation, staff record review, and interviews, the center failed to ensure that each grouping of children had a qualified program lead.
Evidence:
1.Staff # 1 was observed working as a program leader in the toddler room on the day of the inspection. Staff # 1 stated she was the program lead. After review of Staff # 1 record, there was no documentation of the educational requirements and experience for a program leader position.
2.Staff # 3 was observed working in the toddler room as a program lead on the day of the inspection. Staff # 5 stated she was the program lead for the room. Standard #: 8VAC20-780-440-N Description: Based on observation of the infant room, the center failed to ensure that toys or objects hung over an infant?s crib were not for infant over five months of age or infants who are able to push up on their hands and knees.
Evidence:
Observed child # 2 (10 months old) sleeping in a crib. A boppy pillow was hanging over the side of the crib.Plan of Correction: Staff removed the boppy pillow during the inspection, the director will monitor.
Standard #: 8VAC20-780-570-E Description: Based on observation, the center failed to ensure that all prepared infant bottles were labeled and dated.
Evidence:
In the refrigerator in the Infant room A had two prepared bottles that were not labeled with the owner name or dated.Plan of Correction: During the inspection the staff labeled and dated the bottles.
